No. It is the agent that is allowed to act for the Principal and not the agent of the agent (spouse).

Anyway, it is true that the agent constituted by the principal must act according to the mandate of the POA and no other.
A Durable" Power of Attorney, though it enables the Agent to act for the Principal even after the Principal is not mentally competent or physically able to make decisions, the same does not cover medical matter. In CA, specific delegation via Health Care Power of Attorney shall be executed.
